Creating an OJT Form

Create an OJT Evaluation form by defining its basic details and adding questions to assess learner performance.

OJT Form Creation

1. From the Admin Dashboard, select OJT - Evaluation from the Menu.
2. Click + Create Form.
3. Enter a Name and Description for the OJT form.
4. Click Create.
5. On the question creation page, click Add Question to add questions to the form.
6. Add the required questions and configure their options and ratings.
   Question Types provides more information about adding questions.
7. Click Save to save the OJT form.

A success message confirms that the OJT form has been saved successfully.

OJT - Evaluation Columns

Delete a Question

To remove a question from an OJT form:

1. Click the three dots in the top-right corner of the question.
2. Select Delete.
3. In the Please Take a Moment confirmation window, click Yes.

The selected question is removed from the form.
